Record low consumer confidence and tight credit continued to dramatically reduce demand for refractive surgery, but the number of Q1-2009 procedures was somewhat higher than our forecast, suggesting that demand may have reached bottom in Q4-2008.

Responses from surgeons to Market Scope’s Q1-2009 Survey of US Cataract Surgeons indicated that demand for intraocular lens procedures (including cataract, phakic, and refractive lens exchange) grew 2.3 percent in Q1-2009 over demand reported in the same quarter last year.

This edition of our survey report is based on our analysis of 528 surveys received from US refractive and cataract surgeons. More than 38 percent of responding surgeons referred to themselves as combination refractive/cataract surgeons, and more than 41 percent called themselves cataract surgeons. The more than 20 percent remaining limit their practices to refractive surgery.

The worsening economic crisis continues to drive demand for refractive surgery downward. Demand for all refractive surgery was down 39.9 percent in Q4-2008 from Q4-2007 totals.

The combined Jules Stein/ASCRS winter meeting was held February 6 through 9 in Los Angeles and was attended by about 250 mostly local cataract and refractive surgeons and 30 exhibitors. Much time in both the general sessions and educational workshops was focused on three main topics of high interest to surgeons: premium IOLs, challenging cataract surgery techniques and refractive surgery updates. Methycillin-resistant post-LASIK infections and IFIS were also on the agenda.

In Q4-2008, IOL procedures grew 3.2 percent over Q4-2007 tallies. While demand for PC-IOLs was up significantly from Q4-2007, it declined from the previous quarter, and weak economic conditions are expected to constrain demand into 2009.

Weak economic conditions and weak consumer confidence resulting from high gas prices, rising food prices and concerns over high levels of unemployment continued to reduce demand for US refractive surgeries during the second quarter of 2008. Economists forecast continued economic weakness for the balance of 2008 and into 2009, and this troubled outlook is expected to continue limiting demand for refractive procedures.

IOL procedure volume grew 3.2 percent in Q2-2008 when compared with the same quarter last year. Refractive lens exchange procedures grew at a much more robust rate, buoyed by the increasing popularity of presbyopia-correcting IOLs, and phakic IOL procedures also grew briskly.

Collecting customer feedback and taking action to make improvements is a well recognized method for boosting performance. Customer feedback programs have become one of the most important tools for improving performance, and many of the most successful companies, including Apple, Amazon, Cost-Co, Lowes, and Harley Davidson, regularly measure their scores. Results from these surveys are analyzed in detail and these companies all have programs targeted to make continuous improvements.

Market Scope Gathers Guidance from Consultants, Surgeons and Practice Managers
In this issue of Market Scope’s Surgeons’ Quarterly Survey Report, we are taking a look at how refractive surgery practices are faring in the current period of economic uncertainty, as well as what strategies might help practices successfully weather tough economic times.
We asked Marguerite McDonald, MD, a pioneering specialist in corneal disorders and cataract and refractive surgery with Ophthalmic Consultants of Long Island, NY, and Robert Maloney, MD, also a pioneering refractive surgeon and founder of the Maloney Vision Institute in Los Angeles, CA, about their expectations, observations and plans for their own practices. Dan Wilson from the ClearSight Center in Oklahoma City, OK, also participated and brought a practice manager’s perspective to our questions.
In addition, we spoke with two experts in the field of LASIK marketing in order to provide additional context and strategies. Wade Morrison is founder and President of LASIK Ad Network, a full-service advertising agency focused on LASIK and other ophthalmic services. Shareef Mahdavi is founder and President of SM2 Strategic, a consulting practice focused on marketing strategy and communications for laser refractive surgery and other breakthrough medical technologies.

In 2007’s fourth quarter, US intraocular lens surgeries (cataract, phakic, and refractive lens exchange) grew 3.2 percent over totals in the fourth quarter of 2006 and 1.3 percent over 2007’s third quarter tally.

During the third quarter of 2007, better than 96 percent of IOLs were used in cataract surgeries in the US, but refractive lens exchange (RLE) and phakic IOL procedures were increasing steadily.
